Blue Swan Home Care is the first service in Sabah to bring specialist care into the home, sometimes described as specialist-led home care, based in Kota Kinabalu. Founded by John Cham and led clinically by geriatrician Dr Gordon Pang, we coordinate medical and nursing care for elderly and unwell patients in their own homes. Neither. We do not take your parent anywhere, and we are not a maid or companion agency. We bring doctors, nurses and physiotherapists into your parent’s own home, working together as one coordinated team.
Quite a lot that families assume only a hospital can do. Doctor home visits, geriatric and frailty care, palliative and hospice care, nursing and wound care, and physiotherapy are all delivered at home. Our services page lays out each one.

Yes, a clinician comes to the home. Day-to-day visits are doctor-led or nurse-led depending on how complex the case is. A specialist home visit is arranged only in select cases, based on clinical need; the rest of the time our visiting doctors and nurses work to the clinical framework Dr Gordon Pang designed, and escalate for a specialist decision when a case needs one, whether to him or your parent’s own specialist. Our clinical framework sits behind every plan. Day to day, your parent is cared for by the Blue Swan care team: our nurses, visiting doctors and care management team working together. When a case needs a specialist decision, it’s escalated to the right specialist, whether Dr Gordon Pang or your parent’s own. It is his framework, and that specialist input when it is needed, that lets the team care for frail, palliative and dementia patients at home.
Yes. Dementia is a particular focus for Dr Gordon Pang, and it is one of the cases our model is built for. Assessment, day-to-day management and family guidance are all part of the geriatric care we provide at home.
Yes. We look after comfort-focused care for cancer and other life-limiting illness: pain and breathlessness, symptom control, and support for the whole family. It can run alongside a patient’s active treatment. See palliative and hospice care.
Yes. Our nurses, many of them former senior hospital nurses trained in infection control, handle wound and pressure-sore care, tube feeding, catheter care, injections, IV medication and ongoing monitoring. Details on the nursing and wound care page.
Yes. Home physiotherapy is delivered by our physiotherapy team and coordinated with any nursing or medical care already in place. It covers falls, stroke recovery, post-surgery rehab, and neurological conditions such as Parkinson’s. Our founder, John Cham, is a geriatric and orthopaedic physiotherapist, and it’s the field Blue Swan grew from.
The Transition Care Plan is nurse-led, for medically stable patients recovering after discharge, surgery or a procedure. The Complex Support Plan is doctor-led, for ongoing conditions, multiple issues, frailty, or cases that need prescribing and treatment planning. Both are explained on how our care works.
It depends on the plan and the visits your parent actually needs, so we keep figures off the website and walk through them openly during your complimentary care planning discussion. That way you see the real picture for your situation rather than a headline number. We explain fees clearly before any care begins, and there is no obligation after that first discussion. No. Equipment, procedures, medication, consumables and after-hours visits are quoted separately from the care plan, so you only pay for what your parent uses.
Yes. The team is set up to work alongside your parent’s own specialist wherever they are, locally or abroad, and carry the agreed plan into the home.

An after-hours call-out can be arranged when a case needs it. It is quoted separately from the standard plan, and we will talk through how it works for your parent before anything is agreed.
