One of our nurses reaches out to the family and arranges a home visit. We look at mobility, medication, medical history and the home itself, so nothing is missed.
Every plan follows the clinical framework designed by Dr Gordon Pang, our Co-founder, Chief Clinical Officer and State Geriatrician for Sabah. When a case needs a specialist decision, it goes to him or the patient's own specialist. He guides the plan, and we match the right nurses, doctors or physiotherapist to your patient.
You receive a summary of what we found and the plan we have put in place. As care continues, we liaise with you and with the patient's other doctors so everyone stays on the same page.
