Just discharged from hospital? For patients recovering from surgery, a wound, a feeding tube, a fall, weakness, confusion, or a change in medication, our nurses start with assessment and monitoring from day one.

The part of our care you never see

Around half of our work happens before and after every visit: the planning, organising, checking and quiet problem-solving that keeps a vulnerable person safe at home. We call it care management and coordination, and it sits behind every single Blue Swan patient.

The home, set up for care

Hospital beds, oxygen, CPAP machines, drip stands, wheelchairs and walking aids. We source them, arrange them and set them up, so the home is ready and safe before they are needed.

Medication, managed

We coordinate directly with pharmacies, compare prices so families get good value, and keep every prescription filled, correct, and never running out.

Nutrition and supplies

When a dietitian recommends specific feeds or supplements, we organise the supply. Nobody has to hunt from shop to shop for the right product.

The family, kept close

Regular updates and conference calls mean children working away, or living overseas, always know how Mum or Dad is doing, and share in every important decision.

There when it matters

After-hours call-out can be arranged. When something happens late at night, a trusted team responds. Often that is the difference between a calm night at home and another trip to hospital.

Your own caregiver, backed by our team

Employ your own caregiver and we fold them into ours: trained, guided, and linked to our nurses through daily health logs and wound photos, with fast escalation when something looks wrong.

If we only visit once a week, how do we know what is happening the rest of the time?

Because between visits there are trained eyes in the home every day, structured updates flowing back to our care management team, and a clinical framework that tells everyone exactly what to look for. A weekly visit is only a moment; the care around it is continuous.

Every Blue Swan care plan is built on the clinical framework Dr Gordon Pang designed, with a particular focus on dementia. When a case needs a specialist decision, it goes to him or your parent’s own specialist. That framework is what lets us confidently take on the cases many services turn away:

  • Dementia and memory-related conditions
  • Palliative and end-of-life care at home

On the ground, care is delivered by experienced nurses, including former senior hospital nurses trained in infection control, who act as the specialist’s hands and eyes in the home.

Day-to-day visits are doctor-led or nurse-led; a specialist home visit is arranged only in select cases based on clinical need. Our visiting doctors and nurses work to our clinical framework, escalating to him or your parent’s own specialist when a case needs a decision.

Questions families ask about our care

QIs Blue Swan a nursing home or a caregiver agency?

Neither. We do not take your parent anywhere, and we are not a maid or companion agency. We bring doctors, nurses and physiotherapists into your parent’s own home, working together as one coordinated team.

QDoes a doctor or specialist actually come to the house?

Visits are doctor-led or nurse-led depending on the case. A specialist home visit is arranged only in select cases based on clinical need; otherwise our visiting doctors and nurses work to our clinical framework, escalating to him or your parent’s own specialist when needed.

QWhat’s the difference between the Transition and Complex plans?

Transition is nurse-led, for stable patients recovering after discharge, surgery or a procedure. Complex is doctor-led, for ongoing conditions, multiple issues, frailty, or cases that need prescribing and treatment planning.

QIs Dr Gordon Pang directly involved in my parent’s care?

The framework he designed as Co-founder and Chief Clinical Officer sits behind every plan. When a case needs a specialist decision, it goes to him, or to your parent’s own specialist. Day to day, your parent is cared for by the Blue Swan care team: our nurses, visiting doctors and care management team working together.

QCan you coordinate with my parent’s existing specialist, including overseas?

Yes. The team is set up to work alongside your parent’s own specialist wherever they are, and carry the agreed care plan home.

QWhat happens if something goes wrong at night?

An after-hours call-out can be arranged; it is quoted separately from the standard plan.
