Who we are
This website is operated by Blue Swan Academy Sdn Bhd (company number 1515873-K), which provides home care in Kota Kinabalu under the name Blue Swan Home Care (referred to here as “we”, “us” and “our”). The information we collect
We only hold information that helps us respond to you and look after the people in our care. It falls into two groups.
Information you give us
- Enquiry details from our contact form: your name, email address, phone number and the message you send.
- Referral details from our referral form: the information a referring professional shares about a patient and about themselves, which can include clinical notes and next-of-kin contacts.
- Anything you choose to include when you email us or message us on WhatsApp.
Information collected automatically
- Basic server log data that your browser sends, such as your IP address, browser type and language, and the date, time and pages of your visit.
- Cookies and similar files that keep the site working and help us understand, in aggregate, how it is used.
Cookies and analytics
Our site uses a small number of cookies. Some are set by our caching system so pages load quickly. We also use privacy-focused, on-site analytics to see which pages are read, reported only as aggregated figures rather than as profiles of individuals. Our enquiry forms use Google reCAPTCHA to cut down on spam, which means Google may process limited technical data when you submit a form. Most browsers let you block or delete cookies in their settings.
How we use your information
We use what you share to:
- reply to your enquiry and answer your questions;
- arrange and deliver home care, and coordinate it with the other clinicians involved;
- process and follow up professional referrals;
- keep accurate care and business records; and
- meet our legal and regulatory obligations.
We do not sell your personal data, and we do not use it for advertising.
When we share information
We treat your information as confidential and share it only where there is a genuine reason to:
- within our own team and Blue Swan Academy Sdn Bhd;
- with doctors, hospitals or other providers involved in a patient’s care, where this is needed to look after them;
- with trusted service providers who work on our behalf, such as our website host, under a duty of confidentiality;
- with Google, in the limited way described above, when you use a form protected by reCAPTCHA; and
- with the authorities where the law requires it.
How we store and protect it
We use technical and organisational measures to protect the information you give us, including secure hosting and encryption of data while it is in transit. No website or system can be guaranteed completely secure, but we take reasonable steps to guard against unauthorised access, loss or misuse.
How long we keep it
We keep personal information only for as long as there is a clear reason to, for example to provide continuing care, to meet a legal or regulatory duty, or to protect our legitimate interests. Once it is no longer needed, we remove it from our systems in line with our internal procedures.
Transfers outside Malaysia
Some of the services we rely on, such as cloud hosting, and some coordination with specialists or family members living abroad, may involve storing or handling information outside Malaysia. Where that happens, we take reasonable steps to keep your information protected to the standard set out in this policy.
Your rights and choices
Malaysia’s Personal Data Protection Act 2010 (the PDPA) sets the framework for how personal data is handled here. Subject to that law, you can ask to see the personal data we hold about you, ask us to correct it if it is wrong, withdraw a consent you have given, or ask us to stop sending you updates. Children
Our services are arranged by and for adults, usually on behalf of an older family member. We do not knowingly collect personal information from children through this website.
Changes to this policy
We may update this policy from time to time. When we do, we will post the revised version on this page and change the date shown above.
